Online access is available for booking appointments, ordering repeat medications, viewing test results and limited coded information within your medical record.

Online Access to Medical Record

Once your Access to Online Services registration has been completed, you should receive an email with your details of how to set up your account and how to link it to the practice. 

If you use the internet to access information about your care and health records you are responsible for keeping your username and password secure. The practice is not responsible for breaches of confidentiality caused by patients themselves.

Identification Upload

To register your request for online access we need to see a form of Acceptable ID (if not possible, please let us know).

We will not store these documents and we will securely delete / destroy them after our initial verification.

Acceptable Identification: Photo Driving License, Passport, Tenancy agreement, Mortgage statement, Bank statement, Utility bill (date within the past 3 months) etc.
