New Patient Registration
To register with Gloucester Road Medical Centre, you will need to fill in a New Patient Questionnaire and a GSM1 form. We will need details of your name, address and current GP to assist in the prompt transfer of your medical records.
A GSM1 form will also need to be completed for all children and a Declaration of Parental Responsibility completed for children under the age of 16.
We request that you provide photo ID and proof of your current address. These can be shown to staff at the reception desk or electronic copies sent to grmc.reception@nhs.net. For practical reasons we need to ensure that people are who they say they are and check where they live, this will help ensure we match the correct patient details to the NHS central patient registry. Please ensure that you live within the practice boundary (as highlighted here).
We will not be able to offer you an appointment to see a doctor or a nurse before you have registered.
Out of Area Registration
If you are out of our area but would still like to register with us, for example because you work locally, please apply in writing via the Out of Area Registration Policy and Form.
Non-UK Citizens Registration
Non-UK citizens will need to prove their entitlement to NHS care.
